(back) Subject: Re: defib -- AED From: "Wm. G. Chapman" <wchapmn@ibm.net> Date: Sat, 20 Feb 99 13:57:45 -0800   I was not going to get involved in the discussion between Judy and Bud. However, it occurs to me that in the organ business we never find anything to _require_ certification for; and, in Nursing, they seem to find little that does not require education, certification or a license. So; here goes...   For background my wife is a RNC and Legal Nurse Consultant [AALNC]. Presently, she is a Nurse Manager. It takes about 33+ hours of continuing education each year to maintain her present certifications. That training is what allows me to convey the current state of practice and law in California.   Bud has it right. He is referring to "Automated External Defibrillators." In addition, he was talking about the semiautomated variety. These require that a button be pushed to check the rhythm and heart rate. There is another button to shock with; if the machine so advises (VF or VT).   Fully automated units are for use by those of limited training -- family, or those who rarely treat cardiac cases. The semiautomated units are considered safer because they can not automatically cycle through analysis and shock without the final decision left to a human being. Clinical experience does not show a significant difference in outcome between the two types.   In California, the use of either of these is approved and is covered by Good Sam laws. Clinical experience shows that the cautions offered about inappropriate use or bad outcomes is limited to less than 1/10 of 1% of patients. (However, I would caution the use of a cellphone within six feet of the unit during analysis.)   On a more personal note I have had the opportunity to monitor the operation of one of these _cardiac rhythm automated analysis units_ up close and for an extended period of time. These are the analysis sides of what Bud was talking about. In three days of operation it made the following errors: #1 -- "bad or loose lead" (adhesive pad failed), #2 -- "unable to analyze rhythm" (cell phone within four feet -- this resulted in several nurses responding to the caution and warning alarms; boy are CCU nurses quick!), #3 -- "two PVCs back to back" (patient movement from back to side). All in all not a bad track record for a machine.   Grant
(back) Subject: Re: defib -- AED From: GRSCoLVR@aol.com Date: Sat, 20 Feb 1999 18:53:10 EST   Hi List--- Thanks Bud,,and Thanks Grant for the update and rational analysis and testing results on the Defib machine. It was highly educational to me,,,a 60yo organ tech, with some cardiac concerns. I now have a pretty good impresssion on the state of the art in these machines, and the last thing I DIDN't want to hear was, "you must wait till the qualified, trained, licensed, and fully insured person is located and brought to site. Wastes a horrible amount of time in my opinion,,when time is of the essence. (No offense Judy,,,nor any value judgement intended)   Cheers, --Roc
(back) Subject: Larry Schoenstein From: "John Stump" <jajonino@email.msn.com> Date: Sat, 20 Feb 1999 19:54:50 -0000   >In reply to D.J. Radanovich about Schoenstein, I feel I can contribute something to this thread. There have been only a couple of times when I thought I had something worth saying on this list that somebody else had not already said or knew more about than me. However, this time I'm up >early on a Sat. and have time to respond, so here goes.Larry Schoenstein was the West Coast rep for Aeolian Skinner. However, the Schoenstein company was not ever the rep for Aeolian >Skinner. They helped G.Donald put the organ in Grace Cathedral and that was when G. Donald invited Larry (obviously the most gifted mamber of the family) to come and work for A.S. Larry did not work for the family firm while he was the A.S. rep. When A.S. went belly up, Larry helped >the firm out in order to keep busy, however by that time they were nothing more than a tuning and repair outfit, not building any new organs.>   >Schoenstein was the official installer of Austin in the S.F. bay area for years and did much of the releathering and repair work for Austin. The firm was going out of business and the building up for sale when when Jack Bethards bought it and began to establish himself as a builder. He kept the name for recognition sake's , but he has nothing to do with the family hsitory or tradition, no more than modern Wurlitzer electronics have anything to do with the original simply because they bought the name. The original S.F. Schoenstien, Larry's Grand dad, was a German immigrant to S.F. who built very nice tracker aaction instruments worthy of Hook and Hastings. Unfortunately most were destroyed in the Earthquake of 1906 or was it 08. The one or two remaing ones have been elctrified and had their cases sawn in half so they good be split to let the window show. Larry said his Granddad was never the same after having seen his life's work destroyed. The sons (Larry's uncles) built poor imitations of early 20th century Austins and Mollers.   I haven't heard any of Bethrad's recent work, so don't know if he has grown or changed, but his early stuff, sounded like the worst of the 1920 and 30's American stuff. Loud, overblown harsh,. woolly sounding Principals with all articulation removed, hardly worthy of being called French Romatic, IMHO, but then I'm very found of Fisk organs of all vintages, so that lets you know where I'm coming from.   I know this having grown up in the Bay area and worked for the Schoenstein comapny for 9 months to earn money for my masters degree program at Juilliard. I worked for them (when they were nothing but tuning and repair) just when A.S. went out of business and Larry was working for them again. He was a true artist and gentleman from whom I learned enromous amounts of valuable information, being only 20 at the time.   Hope this is not too long and that some of you enjoy this little bit of history.
